We had the same situation. Our 485 had been returned twice. The key point is to pass the mailroom, so the third time our attorney used two envolop, so the mailroom won't miss the message on the cover. He also said if that failed, we woould have to file an I140 amendment to change the Priority day for EB2. Fortunately it passed. Third time is a charm.
The attorney mentioned that he handle other application exactly the same way but ours was the only one returned back.
