1 & 2 Go to http://www.immigration.gov/graphics/formsfee/forms/index.htm, download I-129, fill and have your employer sign it.

3. Yes, same thing. I-129 is the petition to renew your tn.

4. Normally takes 1.5 - 2 months, if you pay additional $1000 premium fee, within two weeks.

5. No or Yes. For No, If you have filed I-129 and pending the result, you cannot leave U.S., otherwise your application will be cancelled.

For Yes, If you get it approved before you leave for China. Alternatively, you can apply TN after you're back from China. The border only do that within two weeks of the expiration date and your new TN starting from the application day for one year, but mail-in application can apply as early as 2-3 months before expiration and will expire on Jan 23, 2005.
