form & your pay stub,you may meet trouble.
You should use your own address, same with your driver license & pay stub, when you move,notify CIS to update
their record.If you have to use a mailing address as a PO
BOX,report CIS your real resident address,but you can ask
CIS to send your mail to PO.BOX address
