1. You surely can only the second result. My doctor sent me a sealed envelop for me to send in to USCIS.

2. My insurance company wouldn't cover this cost since it is not because I was sick. You probably have to pay both. The first doctor 沒給打該打的疫苗 maybe because you already had them all done. I showed proof that I didn't need to have any more 疫苗
