I just applied B2 extension for my mother in law. ted on 10/20/06 and got approval notice on 1/17/07. As for I94, send copy only, even original is required on 539. check out following website for other detail requirements:

My mother in law initially got 3 month when she entered US. We extended it for another 6 months. as long as it's the first time you apply for extension, I believe 6 months is ok.

as for the statement, we wrote it in my mom in law's name. the officer won't know that she doesn't know english any way. we just say she needs more time for trevaling. we also send a copy of her air ticket which is one year open. as for support, we say we'll support her stay.
