h1-b transfer

要班h1-b transfer，
they need the following documents
1. Copy of both sides of the I-94 that are in the passport
2. Copy of visa
3. Copy of passport
4. If currently H-1 all forms I-797

Because I changed my j-1 visa into h-1B in US and have never left the states after
that,. that is, I have not H-1B visa stamp on my passport; So, what is the meaning of 2. copy of visa then. If it refers to
approval notice (I-797), Why then point out I-797 again in NO.4.?
Do they need the original I-797 or just copy?
