I just got back from Beijing. Here are what you need:
1. Your passport.
2. I797 H1B approve notice(original).
3. Company employment confirmation letter(from your HR).
4. recent 3 months pay-stubs.
5. Form 156, 157(US embassy website), 156 need to be fill on line.
6. one two-inch visa photo.
7. application fee 800RMB at ZhongXin Bank.

Those are all I need to get the visa. You may have the following with you:

8. I129(H1B application package).
9. Tax-return and W-2.
10. Degree and trans, I-20.

Don't worry. H1B is very easy.
