It is very unusual now to wait for I140 approval more than one year. Most people got their case approval or RFE in a couple of month. I believe something wrong with your case. One possibility is that your application package can not be found temperally for some reasons. If no one take any action about it, it will lay somewhere for unknown time. My friend's case was pending for half year without any information, so her laywer handled her case through lawyer's special channel (I don't know the detail, probably lawyer association). She received RFE quickly, and her case got approved soon.
It is unwise to wait without any actions. I think a good lawyer should have some channels to communiciate with CIS officer, at least he/she can urge CIS officer to touch your case.
Do not wait. You can call CIS customer service. If you are luck, the customer representative may transfer your call to CIS officer who will tell you what's happening in your case. Alternatively, you can make an appointment at local office via InfoPass online. A nice officer could check your case, most importantly I think if they can find your case in their system. It is not guaranteed to get useful information once, just try and try. Taking action is better than doing nothing.
