Sounds you are qualified well. A lot of times it could be a wording issue but you need to provide new evidences ( such as more references) for sure if RFE is needed. Your don't have to go with every issue that has been raised. For reference letters, I think location diversity is important. Getting international letters and rewording and strenthening your invited talks may help a lot for issue #1. If you don't have an award, better not to mention it. For membership, ordinary membership won't work any more. Few months ago, somebody said Sigma Xi's membership still worked. You might take a look at it.
