Help about medical bill

5 years ago (2001), I owed hospital some medication fee $698 for some reason. At that time, I thought that was hospital’s mistake but hospital insisted that that was not their fault. Finally I didn’t pay hospital $698. As a consequence, my credit is affected by this reason recently. What I worry so much is if this affect my I-140 and I-485 approval?
