I do not think you require another waiver.A waiver for J-1 is approved for your J-1 visa,not for your extension of J-1 program.A J status requires waiver because its J visa has two-year home country residence requirement.It does not require waiver if your J visa without two-year requirement. A key point here is your extending your J-1 program whether or not that will be approved. If it is approved,and you donot get a new J-1 visa, it does not require a new waiver. When you extend your J program, you should tell the administration of J program about the waiver under its processing in the CIS.
