1) form485 and $check
2) 2 passport photo
3) any information included in your passport (homepage, visa, 94 card and all of your H1b visa approval or extension notice, and other visa materials (better color print)
4) medical examination form 693 (must be sealed by your Dr.)
5) form G315
6) recent 2 years tax form and related form (W-2, etc.)
7) (your 140 approval notice. I didn't have)
8) Notarized Birth Certificate
9) Notarized of Marriage Certificate
10) Employment Letter and copy of Paystubs for the last three months (seems that Eb1-a not necessary, but I included them)
