I see. So you do not have any copy to approve the name change and you cannot get a copy of it. You know, each time when you fill in form you need to provide the name and the "new" name (if changed), maybe you can find some evidence of that to approve your case. (The lawyer may has a copy of such evidence when you applied visas to come to USA.)
