Disciss with Howdy

I remembered one of my co-worker ever did it for a client, but she failed. After the I-824 was filed ,CIS
gave a RFE to require the proof of I-485 approval,but at that time,we just got I-140 approval,I-485 is pending,so she couldn't provide it.Finally the case has
been denied, she had to refiled it after the petitioner getting his green card.( he filed the I-824 for his wife & children who were in China at that time.
Do you think was it normal? Why did the situation happen?
