I think the following index maybe more appropriate:
1. Check or Money order
2. Notarization of your English translations and copies
3. Cover letter
4. your awards
5. proof of judge
6. memberships
7. publications
8. citation
9. supporting info about yourcontribution
10. reference letters
11. CV of yours and your referees
12. your phd. degree
13.your passport and H1b...
etc.

Remember, your coverletter should be very well written and should detail all of your solid proofs to prove that you are extroadinary. usually, after the INS reading your coverletter (or personal statement), they should get a very deep impression about you and your achievements.
