Come on, don't be so lazy. Just download the I-765 form, read through the instructions and fill up one single form. Mail the application along with your photos, I-485 receipt copy (no need for I-140 copy at all), photocopy of your passport and expired or still valid visa and the I-97 and whatever I-129 H1 extention if you have.

I just got my EAD approved in 30 days from CSC.
