From H1B to Green Card

In general how long will it take from H1B to Green card. It seems someone can get green cards within two years after arriving the US with H1B. But I also heard that some take several years and some cannot get after 6 years.

My question is on what conditions can one get green card very quickly (say, within two years) after arriving in the US with a H1B?
