Just like apply for a new H1 as everybody else. You do not need any quota any more. There is no difference between H1B and H1B1. You can use premium processing so you can go to work within two weeks (if everything is OK).

What school did you transfer to after H1? Formal school, language school? Did CIS give you any trouble?
