The best option is to sign W-9 form in December and give it to payroll, so your first $5000 will be exempt from federal tax. As for where to fill out the tax treaty info in 1040, I really don't know. Last year I used turbotax to do my tax return, and no tax treaty info was shown in the form. In my W2 the $5000 is not counted as taxable income for federal tax, but counted as taxable income for state tax. So it should be OK.

I guess form W-9 is a new form from IRS. Before IRS asks us to fill out 1040NR if we want to claim tax treaty benefit. W-9 form is obviously a change from that, because it recognizes both resident status and tax treaty.

To answer your question about the statement in W-9, you can use the same wording for form 8233 (eg. I am a student from China on F-1 visa. I am studying toward my PhD. ..... Tax treaty article 20(c) states........)
