how much is medical examination for Canada immigration

called one of the listed doctor in nyc for canada examination. $200 for adult and $100 for kids and by cash only, no medical insurance to cover it. Is it reasonable? also was told that adult should bring additional 3 photos same as the one in the form and one additional photo for kid. This requirement bothers me. How could I get the same photo in the form since i took those one-time photos. Anyone has the similiar experience on that? Thanks
