急問填表，help for filling in forms

Dear brother/sister,

some questions about filling in the forms,
1. i140表中part 4, 第四個問題"if the person's native alphabet is other than Roman letters, write the person's foreign name and address in the native alphabet",應該填漢字的名字和地址，還是填拚音，或者是填 N/A?

2.i485中prat 3 問題2“Have you received public assistance in the United States from any source, including the United States government or anystate, county, city or municipality (other than emergency medical treatment), or are you likely to receive public assistance in the future ",象我原來由於工資少，一個人工作，生小孩申請過medicaid 和food program並被批準和使用過，這個我要選yes並寫上去嗎？

3. i131 part 1中問題3“class of admission", 該如何填？

4. i140中prat 3中"if in the U.S.,current nonimmigrant status",我應該填我現在的非移民身份H1B,但是在 i765表中問題15“curren immigration status (vistor, student,etc.), 由於我現在是非移民身份H1B，所以我應該填 N/A,還是填 H1B,而我lz填 H4?
