A general physical checkup plus blood test for syphylis and HIV, and tuberclosis(sp?).

You can have RPR test, HIV test and chest X-ray done in your primary physician's office and bring these results, which shall be accepted without problem by your chosen INS-certified doctor (you might want to check this out when you make your appointment).

You need to bring your immunization records - MMR, Td, and Varicella are required. It is much cheaper to get these shots in your primary physician's office (possible free of charge) or local health dept.
