B1/B2 Reapply procedures

Can someone explain?

Do the people denied the first have a special interview date, or they are scheduled as other applicants?

Do you need to fill out new DS 156 and 157?

Also, what are the chances of approval on the second try? I know this may be a silly question because it is case by case. But do many people actually get it on the second try?
