I did it in Guangzhou.

1. Your approval notice of H-1b(can not remember the name of the form)
2. Your Diploma
3. Your paycheck stub (last 3 months)
4. The standard visa application forms.

Pls also prepare(just in case)
5. Your resume and a letter from HR stating that your position and job responsibility

6. Business Card.
