Obviously, you did not prepare a good petition letter. Don't assume the officer will read through your supporting materials, you have to make it very clear what you want to show IN THE PETITION LETTER.
All the attached documents, including citations and reference letters are just there for validation, i.e. if they want to see the original piece. But the main informaiton should be in the petition letter and clearly stated, so the officer can just read the letter and get 100% of the idea you want him to have.
