please help with EAD application after advanced parol

I am going to renew my EAD, I485 was filed sept, 27, 2005, and used the advanced parole last month. now I am applying to renew my EAD by I 765, the questions about the form are:

1 form 14: manner of last entry (visitor student, etc):
what should I fill?

2 form 15: current immigration status:
is AOS ok?
