Lost EAD card again!

Hi, everyone,
I got my EAD card a couple of years ago after I graduated with master degree. But I did not work with the card since I applied for PhD program instead.So pretty much wasted the one year OPT. Now I am in the middle of the PhD program and want to apply for H1B Visa. Do I need to submit the copy of the EAD card even though I am currently on F-1 visa? If so, how do I deal with the loss of my EAD card? Your reply is highly appreciated!
