I140 approved, 怎樣準備485材料

雇主替我申請的140 (EB2)，沒有請律師。 現計劃準備485材料。 有如下問題請教：

1 我可以自己 file 485 嗎 （without hiring lawyer)？
2. 是不是 file 485 之前必須體檢？
3 怎樣找體檢醫生 （Atlanta area).
