Here is a free list of materials for EAD application or renewal for the category "C09"
1. Fee payment for I-765 application ($ 180.00, check again to make sure it is the right fee) and fingerprint fee ($70 if you have not done any FP for your 485 yet, if you did FP for 485 already, you need to mention that in the cover letter).
2. One completed form G-28 (not necessary if you do it by yourself, but need when you do it on behalf your dependents)
3. One completed form I-765, Employment Authorization Document
4. One completed form I-864, affidavit of finance and a related document in support of Form I-864 for your dependents, you can support yourself by obtaining an employer letter if you have job.
5. A copy of your I-485 Receipt Notice
6. Copies of your all I-94s form
7. A copy of your last EAD (front & back) and last EAD Receipt Stub if any.
8. Two (2) passport-style color photos
