2. I-485
3. Birth Certificate
4. Copy of passport pages
 Front page
 Copies of visas (two F1 visas and one H1B visa)
5. Two identical photos taken within one month
6. I-693, Medical Examination, with copy of vaccination.
7. Form G-325A (4 copies)
8. Copy of form I-94 (both sides)
9. Employment letter
10. Receipt of I-140 petition


Checklist for application for employment authorization


1. Application fee: $180
2. Form I-765
3. A copy of Form I-94 (both sides)
4. A copy of my last EAD
5. Two passport-style color photos
