it is more safe to get forms from Zhong Xin Bank( in China). My Mom got trouble when she showed the forms I mailed to her. She came out of US Embassy and paid 40 Yuan to get some one to fill the forms again with English and Chinese. This was my experience. Maybe my Mom was not lucky.
