EAD Help!

I got my EAD approved, however, they mailed me a mistaken card with wrong time valid from 03/27/05 to 03/26/06. What a mistake.
I rushed to make a call to Custom Service this morning. The lady on other end told me to fill in again I-765, together with the copy i filed before.
Here i ask help from your guys, what are proper procedures to go through to correct the error? what are the required documents to submit? I don't want to see another mistake happen. How long it will take?
