I-131 Form Questions

Thank Mytwocents and other friends for having answered my questions regarding I-131 few days ago. Bu I still have some other questions and ask for your help.

My situation: I came to US with an F-1 visa few years ago and got a tenure track position after I finished my degree. I filed I-140 and I-485 concurrently in August 2004 through a layer. The I-140 was approved in November 2004. The I - 485 is pending because of known reseaon. Currently I still hold an H1B visa and my wife works with an EAD. We are planning on a trip to visit my parents in this summer.

My Questions in I-131:

1. In part 1, item 3, Class of Admission, Should I fill F-1, or H1B or something else?

2. In Part 2, should I check item a (reentry permit) or d (Advanced Parole)?

3. In part 7, Question 1, should I check One trip or More than one trip?

A general question on I-131 submission:

Where should send the I-131 form, the Service Center where my I-485 filed or any other specific place?
