an notary service would work. In my case, I checked with my international office and was told there was one guy who does notary service. I went there with the original Doctor's note (I have someone scanned that and e-mailed to me), the translation I made for the note and a friend who agreed to be a witness of my translation. That person also need to bring an ID, i.e. Driver's license. And that is it.
