Put in the city where your service center is located. For example, I live in Alabama. But I need to submit my application to Mesquite, TX. Therefore, I fill in item 10 as city (Mesquite), State (TX).
However, you might want to make sure that the form ETA750B is still accepted by INS. Previously in this forum, someone mentioned that this form is replaced by another one. You might want to check that out.
