1. You can come back with a J1 visa. You do not have to comply with the 2-year requirement if coming back with a J-1 . If the program ramains same, should not be a big deal for a new J-1 visa. I know many people got the returning J1.

2. Right now J1 is registered in the SEVIS system. if your boss stop sponshorship, INS knows it.
3.If you switch the program, VO may start doubtting your immigration intent.
4. Rule says that if your visa application is turned down in the third country, you can not come back. However, many people postes succeful experience coming back even their visas are denied. Take your own risk and prepare for the worst.
5. Should be no difference to get a waiver and to start GC in the fouth and fifth year of your J1.
