I submitted application for my hu*****and's EAD, based on the I-485 I submitted for him last Sep. The application was sent to TSC. Within 10 days, we received the card in the mail. That is much faster than what we expected. Maybe too fast since he doesn't need that right now. Anyway, it is good to have a peaceful mind.
Here is the list of document that we used for his application.
Checklist for Application for Employment Authorization (I-765)
1. Form I-765
2. His I-485 receipt
3. My I-140 approval notice
4. Two color photographs with Alien Registration Number (A#) on the back.We put that in a small envelop, and wrote his Alien number outside the envelop.
5. Application fee of $180.00
6. A copy of Form I-94 (front and back)
7. A copy of former employment authorization card from OPT (front and back)
Note:
a. For the form, we filled that out with Acrobat standard version. It can save the information that you fill in. So if you find some mistake after you fill in, you don't need to start over. I know it is not a free version. I used that because my working place have that. If you can get that software, it is better than the free version.
b. for photograph, we took that at home with digital camera and printed out with our cannon photo printer and it works well.
c. For application fee, we sent a personal check payable to Department of homeland.
