Here is my list for I-485.
Principal Applicant:
Document 1 List
1. Form I-485 with a check $385.00 (including I-485 filling fee $315.00 + Fingerprint card $70.00)
NOTE: I think that the price has changed recently. Check out the USCIS website for the current fee.
2. Form G-325A with two photos of the applicant
3. Form I-693: Sealed Medical Examination
4. Copy of Notarized Birth Certificate
5. Certified copy of Marriage Certificate
6. Copy of Notarized No Criminal Offense Certificate from China
NOTE: it might not be necessary. I attached it because I have it.
7. Copies of Passport, Visa Pages
8. Copies of Form I-20 and I-94
9. Employment Letter and copy of Paystubs for the last three months
10. Copy of Income Tax Return Form (1040) for the last three years with W-2 and 1042S Forms
11. Copy of Social Security Card, Driver’s License


Applicant for spouse:
Document 2 List
1. Form I-485 with a check $385.00 (including I-485 filling fee $315.00 + Fingerprint card $70.00)
2. Form G-325A with two photos of the applicant
3. Form I-693: Sealed Medical Examination
4. Copy of Notarized Birth Certificate
5. Certified copy of Certified Marriage Certificate certified by the Sate of XX
6. Copy of Notarized No Criminal Offense Certificate from China
7. Copies of Passport, I-94, EAD card (OPT), Visa Pages
8. Copies of H1B approval notice and new I-94 applied while applicant is in the U.S.
9. Employment letter and copy of Paystub for the last three months
10. Certificate of Joint Bank Account for dependent applicant and principle applicant.
11. Copy of Income Tax Return Forms (1040) for the last three years with W-2 and 1042S Forms
12. Copy of Social Security Card, Driver’s License
