有人用親身經曆證明750B已經作廢了，請用9098。

My 140 package/NIW was returned by USCIS, and a note says:

"You have submitted a form that is no longer valid for the I-140 process (ETA
750). You must submit the new version of this form, which is the ETA 9089. The
new form replaced the ETA750 on March 28, 2005, and is the only version
accepted."

But this ETA9089 is a form that should be filled by the employer. (combination
of ETA750A and B). There are many blanks that I can not fill. Any idea?
