關於結婚綠卡。

我要幫我老公辦結婚結婚綠卡， 上網站查了一下。

我(公民)需要：file form 1-130 and 2 signed G-325A forms , also need submit the copy of cerficate of Naturalization, photos, marriage certificate

他需要：file form 1-485, from G-325A, from 1-693, form 1-864, and form 1-765.

我有幾個問題想請教大家。
1) 我們應該寄這些表格去local INS office, or USINS NEBRASKA SERVICE CENTER. We are in Missouri.
2) 請問去哪裏作Medical Examination，大概需要多少錢，因為我老公現在還是學生,沒有醫療保險。
3) 以上的表格都需要一起發出去嗎？
