Anyone did AC21 process?

My I-140 is approved long time back. I-485 pending for more than 1.5 year already.
I just lose my job and my friend is able to offer me a new job. I think it comes to an issue of using AC21.
Where to find the AC21 form? Who should fire it? Me? or the new company? Or even old company?
