How I fill in these fields,Please advise:

1. I am in the US as
a. Visitor b. Student c. Permanent Resident d. Other ____________ (Specify)


2. Copy number from Alien Card
A ___________________


3. If not a permanent resident, my stay in the US expires on: _______________(Date)

I don't know use H1b or I485 information.
