2. You must come back with physically visa if you go to China. You can apply for visa in US Embassy or Consulate in Canada. Likely you will only get regular H1b visa as non-PR Chinese, which is valid for 3 months, 2 entries for H1, 1 entry for H4, because you don't have Maple Leaf Card at that time.
3. Yes, before visa expiration date.
