I went to Juarez, Mexico. The city is very close to El Paso, TX. First, you need to purchase a pin and make an appointment at http://www.visa-usa.com.mx/. I followed the instruction on http://ciudadjuarez.usconsulate.gov/wwwhnivh.html for all the documents plus additional items. There is another more complete list of documents you need to bring after you make your appointment online and you can print it from the website. After you make your appointment, you need to have your photo taken and have the DS-156 form filled. You need to pay $1000 pesos for application fee (I paid $107 dollars at the bank). You need to pay this fee in person with your passport at the bank, which is very close to the Consulate. When I was interviewed, the visa officer didn't pay much attention to anything I brought but the original I-797 notice and asked several questions. If you were J1 before, make sure you have your waiver letter ready. Once you get the H1B visa, there are two entries within three months. Hope the information above is helpful.
