If you go to USCIS website they have a brand new form (version of Oct. 25 2005). I guess you are under category C9. If you look at the form and the instruction carefully you will see you have to submit to the same local office/service center where you submitted your 485. I did this for wife and we already got the receipt. My friend submitted at the beginning of October and at that time there was another page to show you need to mail it to Chicago based on where you live. Now this information is not available anymore (he double checked for me).
