The following is a list of documents needed for registration:
PROOF OF AGE AND LEGAL NAME
Original birth certificate. If you do not have an original birth certificate you may bring passport, I-94 or any other official document that would prove date of birth of the student. However, if you do not have an original birth certificate you will need to complete and notarize an affidavit (provided at Intake Center) stating the reasons why you do not have an original birth certificate.
PROOF THAT THE STUDENT LIVES IN xxxxxx COUNTY
Rental Agreement/Lease, Property Title, or notarized statements (statements can be obtained at Intake Center) to prove that the student lives in Arlington County.
MEDICAL REQUIREMENTS
Immunization records: polio, measles, mumps, rubella, tetanus, diphtheria, whooping cough. A TB test given within the past year is also mandatory. Elementary and Middle School students must have, in addition to the shots, a physical examination given within the past year. In certain circumstances, the Intake Center will make an appointment with the school clinic. If the student has the physical given by a private doctor, he should fill a blue form which can be obtained at the Intake Center. Students are not allowed to enter in Arlington Public Schools unless he/she has all the required vaccinations, the TB test and, for Elementary and Middle School students, the physical examination.

Social Security Number (if available).
