回複：申請I-485 list

1. A notary copy of Form I-797: The Approval Notice of Form I-140 (or your I-140 receipt). (A bank check attached).
2. This content list.
3. Form I-485, 2 photos attached.
4. A notary copy of Form I-797A: H1b visa with Form I-94 (if you never make H1b extention in USA, maybe form number is different).
5. Form 9003.
6. Form G-325A.
7. Form I-693 for Medical Examination (sealed by doctor).
8. Form I-765, 2 photos attached. (this is for EAD card, it is optional, depend on your decision).
9. A notary copy from the passport to show your birthday and birthplace.

If you have family members, just repeat these forms except #1 and #2
.
Beside these, you should also submit:
10. Notary copy of the Marriage License of you and your spouse if your spouse submit together with you and is your depedent.
11. Notary Authentic Act of the birth of your child if you have child, original Chinese document (if child born in China) with English translation.
12. Employment certification of you, signed by Human Resource of your employer.
13. Three years W-2 forms (Optional, depend on whether you have paid tax, it is useful).
14. The copy of the tax return (The copy of Form 1040, if you pay tax and have submitted tax return each year, you should keep its copy.) #13 and #14 are required for EAD card application.
15. The notary copies of any of your finacial evidence, bank statement, and so on. The more the better.
16. I-864 you signed before the face of a recagnized notary guy, it is for finacial support from you to your H4 spouse and child.
