Please Help 加拿大移民 change address

I recently faxed my change address request to Canada Immigration Regional Program Centre at Bufflo. I heard there is change request form I should fill, but I didn't receive any form, and I would not find the form on the website : cic.gc.ca . Does anyone use this form? Do immigration officer accept my request?
