Except for the application fee and I-131 form, you should also provide the following supporting documents:

1. Two recent photos;
2. A copy of I-485 receipt notice;
3. A copy of H1B approval notice and current I-94 card (both sides);
4. A copy of your driver's license and/or passport identification page;
5. Copies of previous nonimmigrant documents including OPT card, I-94 card, and visas.
