The same thing happened to me four years ago. In my case, I did not send a copy of the new IAP66 form. But one saying is if you received the notice from DOS, you can not do J1 visa extension. My case was I got my J1 extended before I received the notice form ISN (at that time,before 911). I believe you can get correct answer from the international office of your school.
